APPOINTMENTS
To make an appointment with us,
call 318 212 8675.
The office goes by appointment
only. We do no accept walk-ins new patients or follow-up patients. If you have a
true emergency proceed to the ER. It is very important that if you are a new
patient, please come 20 minutes before your scheduled time. If you are late for
your appointment, please call and check with our office if we can still see you
that day. We will make every attempt to accommodate you. When you have an
appointment with us, we will make every attempt to see you at your scheduled
time, but due to the fact that Dr. Sikand provides emergency neurological care
to patients at local hospitals, and some patients’ visits take up extra-time,
you may have to wait past your scheduled time. In the case that you have to wait
past your scheduled time, we hope you will be patient with us, as we will give
you the same unrushed attention on your visit with the doctor. We understand
that your time is valuable, and in the event you cannot wait, please let the
receptionist know and we will reschedule your appointment.
CANCELLATIONS
If you are unable to make your
appointment with us, please notify us 24 hours in advance. This allows us to see
other patients who are on our urgent waiting list. If your medical records
reflect that you repeatedly do not show for your appointments we reserves
the right to terminate you from our practice. If we can help make your visit
easier and more comfortable, please do not hesitate to call us. After all, we
are here to provide you with the best care. You are our patients and our first
priority and will we try to accommodate you in anyway possible.
We do not see Worker
compensation, motor vehicle accidents or accidents of any type where litigation
is either underway or contemplated.
PRESCRIPTIONS AND REFILLS
Prescriptions will be given to
the patient at the time of service. Please make sure that if you are low on your
medications, ask the doctor at the time of your visit. If you have a mail in
pharmacy, we would be happy to fax your prescription to them. The patient should
allow at least 24 hours notice when in need of a medication refill. Refill
requests should be called directly to your pharmacy and they will in turn call
this office if no refills are left on the prescription. If your request for
refill is received by 2pm, Monday to Friday, we will call in the prescription
the same day by 6 pm. Please keep in mind that if you have a neurological
disorder and are on treatment, Dr. Sikand requires that you be seen in the
office every six months for a follow-up appointment, unless otherwise instructed
by Dr. Sikand. Because the on call doctor does not have access to your records
when the office is closed, no pain or headache medications will be prescribed at
nights or on weekends. If you are in sufficient pain that you need medication,
then you will need to go to Quick Care or emergency room to be treated.
TEST RESULTS
If you have testing done, the
results of your tests will be given to your physician to read and interpret.
Once the results of ALL your tests are available, he will go over these results
with you at the time of your follow up appointment. If your test results are
normal, you will not hear from us in advance, however; if something abnormal
shows on the tests that needs immediate attention, be assured the physician or
nurse will contact you right away.
In addition, for certain
neurological diseases, we may not communicate the results over the phone since
Dr. Sikand prefers to discuss the results with you in person, as it gives you
and your family an opportunity to ask any questions and discuss the treatment
options.
TELEPHONE CALLS
Please call us Monday to Friday
at 318 212 8675 with any questions regarding your medical condition,
medications, medical records, billing or anything else related to your care.
Telephone calls received by 2 pm will be returned the same day. Remember, if you
had any tests done, we only notify you over the phone if the results are
abnormal and require immediate attention. If your tests are normal we will
discuss those results with you on your next visit. If you have certain
neurological conditions, Dr. Sikand will personally discuss those results with
you. You are welcome to call us if you are concerned about the results of any
test you had, and cannot wait for your follow up appointment.
All medical-related telephone
calls are submitted to Dr. Sikand for review. When you call our office, please
leave a concise concern or message for us to review. After review by Dr. Sikand
or the on call physician, our medical staff may return some calls. Please do not
call and say " I want to discuss something with the doctor and no one else" If
you need to speak with the physician, he will return your call when the
situation permits. The staff is also available to answer any questions you might
have concerning appointments, billing or forms.
EMERGENCIES
In the event you have an
emergency, please call 911 or the emergency telephone number in your area or go
immediately to the nearest emergency room. The emergency room physician will
contact our physicians if needed.
MEDICAL RECORDS POLICY
To request medical records,

Please be advised that we do
not release medical records without a current signed consent for release of
medical records from the patient. Please review our Notice of Privacy Policy.
If you are requesting medical
records be sent to another physician there is no charge. Please provide us with
written authorization to whom you want the records sent. Record will be faxed if
less than 20 pages, or mailed. In order that your records get where you need for
them to go in a timely fashion, we request that you allow us 48-72 hrs. There is
a charge to releasing records to law firms and other entities. If you need your
medical records released, please fill authorization to
disclose protected health information.
If you need to designate a
family member for release of medical information, please fill Designation for
Release of Medical Information to a Family Member, Friend or Legal
Representative.
PAYMENT/INSURANCE
POLICY
We will file
your bill to your health insurance. You are responsible for knowing your
policies' coverage requirements and notifying us of any changes. . Patients are
required to provide proof of insurance at the time of service. Any deductibles
or coinsurance amounts due by you will be invoiced to you once your insurance
has responded. Co-payments for HMO’s, PPO’s and other managed care plans must be
paid at the time of service.
Billing patients for their co-pays is a violation of many managed care contracts
and will not be allowed. For your convenience, we accept cash, checks,
MasterCard and Visa. If the
patient is unable to pay at the time of the visit, the patient may reschedule
the appointment in order to meet the co-pay requirement.
There is a $35 service charge for returned checks. After receiving two returned
checks, we will only accept cash or credit card as payment for future balances.
If you need financial assistance or have questions, please contact our Billing
Department. If you fail to meet financial obligations, your outstanding balance
will be sent to a collection agency and you will be required to pay the entire
amount plus any collection agency fees before being scheduled for any further
appointments. Overpayments will be refunded after all charges have been
processed and paid by your insurance company. A refund check will be written and
mailed within 30 days of your written or verbal refund request.
